PREPARING FOR YOUR VISIT
To help us better understand your problem, we have created a form that will collect important information. You can download a copy of the history form by going to the Patient Forms Page, or CLICKING HERE.
It would be very helpful if you can answer as many of the questions as possible. It is important that you answer the questions yourself - rather than having someone (your partner, spouse, or companion) fill out the form and answer for you. Please fill it out the best you can. We will review your answers during your visit, and this information will be very useful to us.
If you have had any of the tests or X-rays listed below, it is very important for you obtain copies of of films or CD's. Actual films and CD’s (discs) are much more helpful than paper reports. Dr. Berkwits will want to review your MRI's and X-rays, as this information will help him provide you with better care.
If we need to look at a film that is not available to us at the time of your visit, your treatment may need to be delayed until we are able to get the film. Having all the information together at the time of your visit helps us practice better medicine and also helps to avoid delays in your care.
